In most instances, your general dentist will refer you to Dr. Nakata for an evaluation and/or treatment of a specific tooth or area.  Before that happens, you'll probably have questions for us, and we'll need some information from you.  We've created this website to hopefully accomplish much of that interchange.

We highly recommend that you pre-register online, and submit your personal data and health history before your scheduled appointment, all in the comfort of your own home.  This will significantly shorten the registration process upon your arrival, and your overall treatment experience will be so much more efficient.  Rest assured that your information will remain protected and confidential.  After you've pre-used this website, you can begin the process by clicking on "Patient Login" at the top of the page.  Simply enter your user ID and password to gain access to the patient registration site (our office will provide you with these codes).
